07/18 Let's Get Physical at FPC Eustis

We will be offering FREE Sports and Back to School Physicals for kids Kindergarten to 12th grade. Registration is not required but is strongly recommended, as space is limited. Walk-ins will be accepted as availability allows. Please choose an appointment time below of 9AM, 10AM, or 11AM and arrive within that hour. Please be kind and patience with us as it is our desire to serve and care for everyone. Registration will remain open as long as space is available.
